SERVICE COORDINATION
Service Coordination (formerly known as Case Management) is a state-mandated program designed to assure that each person recieves the correct services, at appropriate intervals, for the duration necessary to achieve the desired outcome. Most of the children and adults seeking assistance from Russell Nesbitt Services have very complicated service needs. This program provides the direction, corrdination, and oversight of all services provided for each individual.
Specifically Service Coordination is designed to assess what each person wants and needs, gather input from others interested and involved in the life of the individual, lead meetings to formulate programs addressing the needs and desires of each individual, conduct ongoing assessments of services, and make adjustments as needed. This program is also responsible for maintaining eligibility for state and federal payment programs.
